What every surgeon should know about Ceramic-on-Ceramic bearings in young patients

چکیده
Based on the exceptional tribological behaviour and on the relatively low biological activity of ceramic particles, Ceramic-on-Ceramic (CoC) total hip arthroplasty (THA) presents significant advantagesCoC bearings decrease wear and osteolysis, the cumulative long-term risk of dislocation, muscle atrophy, and head-neck taper corrosion. However, there are still concerns regarding the best technique for implantation of ceramic hips to avoid fracture, squeaking, and revision of ceramic hips with fracture of a component. We recommend that surgeons weigh the potential advantages and disadvantages of current CoC THA in comparison with other bearing surfaces when considering young very active patients who are candidates for THA. Fracture of ceramic components as a serious failure after total hip replacement with ceramic-on-ceramic bearings
Introduction Ceramics have been introduced in total hip arthroplasty for several advantages including minimal friction, low wear, biocompatibility, no ageing problems and no problems related to the systemic increases in serum metal concentrations. In the first generation of ceramic components introduced in the 1970’s the risk of fracture was substantial, reaching up to 13.4 %. After introducing...
متن کاملSurvival of ceramic bearings in total hip replacement after high-energy trauma and periprosthetic acetabular fracture.
Surgeons remain concerned that ceramic hip prostheses may fail catastrophically if either the head or the liner is fractured. We report two patients, each with a ceramic-on-ceramic total hip replacement who sustained high-energy trauma sufficient to cause a displaced periprosthetic acetabular fracture in whom the ceramic bearings survived intact. Simultaneous fixation of the acetabular fracture...
متن کاملSYMPOSIUM: UPDATE ON HARD-ON-HARD BEARINGS IN HIP ARTHROPLASTY Survival of Hard-on-Hard Bearings in Total Hip Arthroplasty
Background Improvements in prosthetic materials, designs, and implant fixation for THA have led to bearing surface wear being the limitation of this technology. Hardon-hard bearings promise decreased wear rates and increased survival. However, there may be different survival rates based on bearing materials, manufacturing technologies, and femoral component designs. Additionally, survival rate ...
متن کاملContemporary Ceramic Total Hip Arthroplasty in Patients with Cerebral Palsy: Does It Work?
BACKGROUND Adult patients with cerebral palsy (CP), who have advanced degenerative arthritis of the hip, have been treated with resection arthroplasty and arthrodesis. Although total hip arthroplasty (THA) has also been used as one of the alternative options, there are few studies on contemporary bearings used in THA. Therefore, we evaluated the results of the contemporary ceramic-on-ceramic TH...
متن کاملAre PEEK-on-Ceramic Bearings an Option for Total Disc Arthroplasty? An In Vitro Tribology Study
BACKGROUND Most contemporary total disc replacements (TDRs) use conventional orthopaedic bearing couples such as ultrahigh-molecular-weight polyethylene (polyethylene) and cobalt-chromium (CoCr). Cervical total disc replacements incorporating polyetheretherketone (PEEK) bearings (specifically PEEK-on-PEEK bearings) have been previously investigated, but little is known about PEEK-on-ceramic bea...
